In a poorly thought out media strategy, Marvel Comics over hyped this issue. That said, it is still an excellent and fitting commemoration of the landmark of hitting 600 issues of Captain America comics (not including the 78 WWII issues).
The issue opens with a familiar sight, the Captain America "Origin" that was first published in 2002. Written by Paul Dini with gorgeous art by Alex Ross. It's a short (two page) explanation of who Captain America is.
From there we jump into "One Year Later", a story around the events of the one year anniversary of the death of Steve Rogers (Captain America). He died two and a half years ago, in the real world, but comic time is very subjective. The story is told in multiple vignettes, each from the perspective of a different character. For example, the first is that of Sharon Carter. In fact, Sharon Carter bookends the story as she is the character that makes the revelation that leads into July's "Reborn" mini-series.
The book also includes a reprint of a 1940's Captain America story from Stan Lee and Al Avison.
